Please mail a check including 2 monthly installments and your $25 annual registation fee within 2 days of submitting this form. Tuition pricing details can be found on the tuition page. 1 hour classes are billed at $58/monthly installment.

Classes are filled on a first come, first served basis. If your desired class is full, we will contact you to discuss alternatives. Classes are full when we reach 12 students. Submission of this form will hold your child's spot for 2 business days. If payment is not received by the 3rd business day, the spot may be forfeited.
